Rheum Without Walls was born from her unique vision and desire to continuously move in the direction of patient-centric care, transparency in health care, and bridging health in-equities. Dr. Salahuddin completed her fellowship in rheumatology in 2015 at David Geffen School of Medicine at the University of California, Los Angeles. I am Dr. Carmen E. Gota, MD, an experienced internal medicine, rheumatology, and weight-loss specialist and the founder of SlimWell Health. I am double board-certified by the American Board of Internal Medicine and Rheumatology and care for adults aged 18 and older.
Here are a few facts about me.
I was born in Transylvania, in Romania. I completed my medical training at Iuliu Hatieganu University of Medicine and Pharmacy in Cluj-Napoca, România, where I graduated as a valedictorian. After graduation I did an Internal Medicine Residency at my alma matter school and joined the University staff as an Assistant Professor of Internal Medicine. In that capacity I taught medical students the discipline of semiology, or how to recognize the signs and symptoms of diseases.
By an unexpected twist in my life, as it often happens, I came to America and had to start all over again. I completed my Internal Medicine residency at Lenox Hill Hospital on the Upper East Side of Manhattan in New York City. There I learned how different the American healthcare system was from the one I just left. I also noticed that being curious, kind and compassionate matters the same no matter where you practice medicine.
Because I was interested in arthritis and autoimmune diseases I decided to continue my training in rheumatology and I completed a two year rheumatology fellowship at Cleveland Clinic Foundation.
The two years I spent there were formative for who I became as a physician. The program was rigorous and demanding as our mentors were accomplished physicians who cared about finding what was really wrong with the patient. I loved the intellectual curiosity and the open minded approach so much that after graduation I joined the Cleveland Clinic faculty. As a Cleveland Clinic rheumatology staff for 16 years I had the privilege to take care of thousands of patients with various rheumatic problems from simple to complex both in clinic and hospital settings. As a member of the Cleveland Clinic Center for Vasculitis Care and Research I took care of patients suffering from various types of vasculitides, which are some of the most complex and challenging conditions in rheumatology. I also had the opportunity to join the Case Western Reserve and Cleveland Clinic Lerner School of Medicine as an Assistant Professor of Medicine. In my teaching capacity I helped train many generations of students, residents and fellows how to think critically, and recognize and manage properly rheumatic conditions.
Throughout all these years of practice I also struggled with the frustrations related to the limits of our care. I felt something was missing and that often we were not addressing the root causes behind our patients problems. For example I wondered about the wisdom of treating arthritic knees without treating the disease of obesity which caused the problem in the first place.
So I decided that I need to learn more about why people gain weight and how to help them lose it. This is how I developed a passion for Obesity Medicine. I discovered that by treating the disease of obesity we can achive major and significant health results. By losing 5-10% of body weight, we can help improve or even cure in some cases diabetes, high blood pressure, high cholestherol, improve mobility and eliminate arthritis pain, cure sleep apnea and many others.
I always believed and tried to help my patients comprehensively. I do not support the sliced and limited problem approach in health care. That is why I kept my Internal Medicine board certification active and continued to help my patients with the management of high blood pressure, high cholesterol, fatty liver, sleep apnea, depression and other medical conditions. As obesity is the root cause for many of these conditions as well, I realized that managing weight could be the key to more effective comprehensive health care for a lot of my patients.
SlimWell Health is the product of this vision for a healthcare model that delivers comprehensive integrative care by targeting weight loss with the goal of major downstream health benefits.

Tara Rizvi, MD, at Houston Rheumatology and Arthritis Specialists in Katy, Texas, is a board specialized rheumatologist with a passion for patient-centered, evidence based care, who treats men and women with rheumatologic diseases in the greater Houston area.

As a speaker and expert representative for the Sjogren’s Syndrome Foundation, Dr. Rizvi was at the forefront of developing a world renowned multispecialty Sjogren’s clinic at the Baylor Clinic. She has helped develop and test innovative learning tools for patients with lupus, lupus kidney disease, rheumatoid arthritis, osteoarthritis, osteoporosis and sjogren’s syndrome.
Dr. Rizvi’s approach to her patients is driven by her passion for patient centered care. She prioritizes spending time educating patients about their autoimmune disease, and believes that empowering patients and their loved ones with knowledge about their condition leads to much improved outcomes. Dr. Rizwan graduated from Hamdard University in Karachi, Pakistan in 2003 with honors. He completed his internship and residency in Maryland, at University of Maryland Prince George's Hospital Center and then completed Rheumatology fellowship at St. Louis University School of Medicine, Division of Rheumatology.
Dr. Rizwan has since won multiple awards for his dedication and care and is known for his compassion and keen ability to answer patient questions in depth. He comprehensively evaluates patients and listens to each of their concerns throughout their treatment processes. To achieve the best possible outcomes, he offers multifaceted treatment plans, including appropriate prescription medications to help alleviate patients’ discomfort.
Natalie MacLean, MD, MSc, FRCPC, is an experienced rheumatologist at Kaplan MacLean Rheumatology in Encinitas, California. She provides specialized care to patients with arthritis, osteoporosis and all other rheumatologic conditions.
Dr. MacLean received her medical degree from the Schulich School of Medicine and Dentistry at the University of Western Ontario. She completed her residency in Internal Medicine and Fellowship in Rheumatology from the University of Alberta. She is a Fellow of the Royal College of Physicians and Surgeons of Canada in Internal Medicine and Rheumatology. She is also a member of the American College of Rheumatology and the American College of Physicians.
Dr. MacLean holds her undergraduate Honours Degree in Biochemistry from Mount Allison University and her Masters of Medical Sciences - Oncology from the University of Alberta. She is a professional microscopist and worked in biomedical research at the University of Victoria and the University of California San Diego prior to her medical training.
Before forming Kaplan MacLean Rheumatology, she was Clinical Lecturer and Rheumatologist in the Division of Rheumatology at the University of Alberta, where she enjoyed teaching students and residents. Dr. Rachel Kapelow is a native New Yorker who completed most of her education in the Big Apple. After an internal medicine residency at Jacobi Medical Center/Albert Einstein College of Medicine in the Bronx, she completed her rheumatology fellowship at Hofstra University at Northwell Health.
During her training, Dr. Kapelow authored many peer-reviewed articles, specifically in topics related to autoimmune conditions and inflammatory arthritis. She has extensive research experience in evaluating the efficacy of ultrasound-guided injections and preventing osteoporosis fractures. Dr. Kapelow is an advocate for her patients, helping them navigate through the most challenging illnesses. She is grateful for the myriad of state-of-the-art treatment options available to help her patients get back to leading productive lives.
Growing up in New York City, Dr. Kapelow enjoyed visiting local galleries and museums. Painting is a fantastic outlet for her to unleash her creative side. As a professionally trained ballroom dancer, she understands how arthritis can affect a person’s life and is dedicated to improving her patients’ day-to-day living.
Leaving the blistering winters behind, Dr. Kapelow, her husband, and their two sons are soaking up the beautiful South Florida weather and local activities. She loves finding time to use her culinary talents to prepare unique dishes blending her Russian heritage and love of multi-ethnic cuisines.

Dr. Matthew Lempel is a Yale University-trained rheumatologist who began his academic training at Yeshiva University and then New York Medical College. He was subsequently pulled into the orbit of academic powerhouse Yale University where he spent six years completing his internal medicine residency, chief medical residency and rheumatology fellowship training. His tenure as chief resident at Yale was a one-year commitment; this rigorous opportunity reinforced his skills as a master clinician and a mentor to newer physicians.
Dr. Lempel’s inquisitive nature and enthusiasm for learning about the history of medicine and the development of new therapies inspired him to become a doctor. His interest in treating complex inflammatory diseases in combination with other medical conditions motivated him to become a rheumatologist. He yearns to help patients with their chronic conditions by engaging them in their healthcare journey and coordinating care with other specialists and their care team.
After a lifetime of living in New Jersey and only spending summer Sundays at the beach, Dr. Lempel excitedly donated his family’s winter jackets to the VA Hospital. The Hirsh Center’s dedication to providing precision joint care through state-of-the-art imaging techniques and compassionate healthcare combined with the South Florida lifestyle were the catalyst for Dr. Lempel’s move. An avid athlete, Dr. Lempel enjoys playing basketball and tennis, running and spending time with his wife and three children in the beautiful Florida sun.

Tarek Kteleh, MD, established Rheumatology of Central Indiana in 2018 with the vision of providing patients with more compassionate and individualized care that’s often lacking in today’s health care community. Rheumatology of Central Indiana offers comprehensive arthritis care at six locations in Muncie, Richmond, New Castle, Greensburg, Shelbyville, and Marion, Indiana.
Prior to establishing Rheumatology of Central Indiana, Dr. Kteleh provided care in partner with Medical Consultants and Indiana University Health. Dr. Kteleh’s provision of exceptional patient care and support has led him to be the leading provider of rheumatology therapies in Muncie and surrounding areas.
Dr. Kteleh received formal internal medicine training at the University of Missouri in Columbia and Kansas City in 2003. He received his rheumatology fellowship training in 2006 at the prestigious school of the Midwest, Washington University in St. Louis, Missouri. Dr. Kteleh practiced rheumatology in North Dakota for two years prior to becoming a Hoosier in 2010.
Dr. Kteleh believes it’s his pleasure to relieve patients’ arthritis pain and help with their disease. He promises patients compassionate and individualistic care that includes carefully listening, explaining the diagnosis and best treatment options, and answering all questions. Patient satisfaction is very important, and his team strives to do everything possible to make them satisfied as well as healthy.
