She has over 30 years of experience in the field of mental health and looks forward to offering compassionate care to those in the greater Louisville metro area.
Professionally, Hazel wears many hats; she is a licensed clinical alcohol and drug counselor, a certified marriage and family therapist, peer support specialist, domestic violence batterer intervention provider (BIP), an ordained minister, and a moral reconation therapist (MRT). Hazel started her education in social services at Peralta College in Alameda County, California. After earning her associate degree, she attended California State University, East Bay in Hayward, California, for her bachelor’s degree in sociology and her Master of Science degree in educational psychology. She started Bridge the Gap Addiction and Mental Health Services, PLLC, in 2004.
Throughout her career, Hazel has established herself as a highly qualified provider of rehabilitative services for substance abuse, domestic violence, and numerous mental health diagnoses.
She offers compassionate counseling services in group and individual settings and facilitates intensive outpatient treatment programs and residential treatment. She also oversees recovery programs, including Alcoholics Anonymous (AA).

In her personal life, Hazel is a mother to two children and a grandmother of eight. She is also active in her church as an assistant minister. She is currently working on expanding her practice to California while also working on new projects here in Louisville.

I am a traditionally trained Psychiatrist, with double Board Certification status. After completing medical school and a first residency in psychiatry in Egypt, my journey brought me to the State University of New York, Downstate Medical Center, in Brooklyn, where I completed my four-year psychiatry residency training program. I subsequently joined the Yale University School of Medicine Law and Psychiatry program where I completed my fellowship in Forensic Psychiatry. My eclectic professional experience included working as a volunteer psychiatrist in Egypt with African and Middle Eastern refugees, survivors of torture and sexual traumas, working with 9/11 survivors and first responders during my years in New York, as well as LGBTQ+ refugees and asylum seekers in both the Middle East and the United States. M Rameen Ghorieshi, MD, MPH, of Palo Alto Mind Body in Palo Alto, California, cares for patients from the paradigm that mental and physical health are so strongly intertwined that one may not fully achieve either without striving for both. Michelle Kluczinske, PMHNP-BC, is an experienced psychiatric and mental health nurse practitioner at Ocean Psychiatry in Freemont, California. At the practice, Michelle offers compassionate and effective care to every patient she sees.
Michelle has experience in psychotropic medication management in outpatient and long-term care settings. She worked with patients 16 years old and over who experienced depression, anxiety, bipolar disorder, PTSD, ADHD, and psychosis.
