I am a traditionally trained Psychiatrist, with double Board Certification status. After completing medical school and a first residency in psychiatry in Egypt, my journey brought me to the State University of New York, Downstate Medical Center, in Brooklyn, where I completed my four-year psychiatry residency training program. I subsequently joined the Yale University School of Medicine Law and Psychiatry program where I completed my fellowship in Forensic Psychiatry. My eclectic professional experience included working as a volunteer psychiatrist in Egypt with African and Middle Eastern refugees, survivors of torture and sexual traumas, working with 9/11 survivors and first responders during my years in New York, as well as LGBTQ+ refugees and asylum seekers in both the Middle East and the United States. After graduating from Yale, I was appointed as an Assistant Clinical Professor of Psychiatry at the Yale School of Medicine.
Hazel Parrish, LCADC, ICADC, BIP, is a licensed alcohol and drug counselor, author, and the founder and owner of Bridge the Gap Addiction and Mental Health Services, PLLC, in Louisville, Kentucky. She has over 30 years of experience in the field of mental health and looks forward to offering compassionate care to those in the greater Louisville metro area.
Professionally, Hazel wears many hats; she is a licensed clinical alcohol and drug counselor, a certified marriage and family therapist, peer support specialist, domestic violence batterer intervention provider (BIP), an ordained minister, and a moral reconation therapist (MRT). She is also a member of the International Certificiation and Reciprocity Consortorium (IC&RC).
Hazel graciously offers mobile visits and telehealth services to her clients while also providing a safe space for them to visit in her office.
Hazel started her education in social services at Peralta College in Alameda County, California. After earning her associate degree, she attended California State University, East Bay in Hayward, California, for her bachelor’s degree in sociology and her Master of Science degree in educational psychology. She started Bridge the Gap Addiction and Mental Health Services, PLLC, in 2004.
Throughout her career, Hazel has established herself as a highly qualified provider of rehabilitative services for substance abuse, domestic violence, and numerous mental health diagnoses.
She offers compassionate counseling services in group and individual settings and facilitates intensive outpatient treatment programs and residential treatment. She also oversees recovery programs, including Alcoholics Anonymous (AA).
After detailed assessments, Hazel guides patients through the process of recovery with a holistic and integrative approach.
In her personal life, Hazel is a mother to two children and a grandmother of eight. She is also active in her church as an assistant minister. She is currently working on expanding her practice to California while also working on new projects here in Louisville.

Michelle Kluczinske, PMHNP-BC, is an experienced psychiatric and mental health nurse practitioner at Ocean Psychiatry in Freemont, California. At the practice, Michelle offers compassionate and effective care to every patient she sees.
Michelle has experience in psychotropic medication management in outpatient and long-term care settings. She worked with patients 16 years old and over who experienced depression, anxiety, bipolar disorder, PTSD, ADHD, and psychosis.
Her nursing experience includes working in inpatient psychiatric units, a psychiatric emergency department, psychiatric outpatient clinics, day programs, substance rehabilitation, and interventional psychiatry. Michelle has also been a clinical educator for the psychiatric-mental health rotation at nursing colleges.
Michelle believes in empowering patients through education and encouraging them to be active and collaborative in their care. She takes the time to really listen to her patients and partners with them to improve their health.

I am a doctorate prepared-nurse practitioner holding dual certifications in psychiatry and family medicine. I hold a master degree in Health Care Management from Johns Hopkins University. I have also received my certification in Transcranial Magnetic Stimulation (TMS) from Duke University School of Medicine. I seek to help my clients achieve their optimal state of wellness--mind, body & spirit, by using individualized evidence-based holistic approaches. I specialize in the management of mental disorders of adolescents ages 14 to 18, adults ages 18 to 65, and elderlies 65+. I combine conventional medication management with psychotherapy, psycho-nutrition (including daily intake of brain booster vitamins. My experience includes treating depression, anxiety, bipolar, spectrum disorders, PTSD, psychosis, ADHD, Addiction, stress, fibromyalgia, pain psychology, and insomnia. Dr. Jennifer Guo is a board-certified psychiatrist with a passion for exploring innovative treatments for mental health conditions. Holding both an MD from Yale School of Medicine and a PhD in Neuroscience, Dr. Guo’s journey into the realm of psychedelic medicine began during her residency at UCSF, where she witnessed the profound healing power of these substances from the lived experience of her patients and the promising research on psychedelics for treating depression, PTSD, addiction, and anxiety.
After completing her residency, Dr. Guo pursued additional training in psychedelic-assisted therapy and interventional psychiatry, working closely with leading practitioners and educators in the field. She started and grew one of largest esketamine treatment programs in the country, and continues to advocate for the responsible integration of psychedelics into modern psychiatry. She has since become a sought-after speaker, educator, and mentor, and remains a volunteer assistant clinical professor at UCSF.
