About Rachel Jiang, FNP-BC

Rachel completed her undergraduate and master’s degrees in nursing at the University of Illinois. She is nationally certified as a family nurse practitioner by the American Nurses credentialing center (ANCC) and she is also a current member of the California Association of Nurse Practitioners. She has been working in health care for over 15 years. Family medicine is her passion, because she wants to help people find ways to healthier and longer lives. As a nurse practitioner, she has cared for a diverse group of people across the lifespan, from young to old.

She manages chronic and acute conditions such as hypertension, hyperlipidemia, COPD, CHF, and diabetes, as well as episodic illnesses. She also manages patients with psychiatric illnesses such as depression and anxiety. She provides annual wellness exams for women, including Pap smears and breast screenings, and educates female patients on birth control and hormone replacement therapy. She also performs school physicals and adult yearly physicals. She understands that people’s cultural beliefs and values are deeply interwoven into their physical and emotional health. She is fluent in English and Chinese. She believes in the value of building a strong and trusting relationship with her patients and their family members. She wants to be her patients’ advocate and partner when it comes to their health and overall wellbeing.
