I have been to other physical therapy places before this visit . I went in skeptical but left very happy . Chad is very good at what he does . I like the one on one time he gives each patient. You dont feel like a number but a real person . He really cares how you feel . I am half way done with my visits and am amazed at how far I ve come with Chads help. I highly recommend Pro Form physical therapy to anyone.
Chad is a wonder worker. Professional, caring and knows his PT stuff. My 30-year ankle injury has never felt better! I now have mobility and stability and I wish I had started working with him earlier. Highly recommend Chad Burnham. If you have a pain, you don't need to live with it!

The clinic is conveniently located. It is modern and well outfitted with all the equipment one would expect to find, and more. Dr. Chad is very knowledgeable and communicates well with my other medical providers. He has been able to motivate me to push through some painful parts of the recovery process and I am regaining full function sooner than I expected.
The clinic is clean, tidy and welcoming, and personnel are professional and friendly. The dedicated attention and education I receive during visits, along with reinforced exercises at home, has contributed greatly to my progress.
