Practice statement

At CHAP Med in Sherman Oaks, California, family medicine and osteopathic medicine specialists along with our highly skilled team, provide comprehensive primary care services to men and women of all ages. Our office is adjacent to Sherman Oaks Hospital, and covered parking is available in our lot.

Our providers are dedicated to dispensing comprehensive care, including preventive medicine and chronic disease management. We also treat acutely ill, hospitalized patients. Rather than focusing on Western or alternative medicine, we take a holistic approach to care, emphasizing tranquility and balance between the mind, body, and spirit.

We’re affiliated with respected medical centers in the area, including Providence Medical Centers, Cedars-Sinai Medical Center, and Barlow Respiratory Hospital. This allows us to provide the full scope of diagnostic services and personalized treatments.

Our CHAP Med experts provide services in our office and at many skilled nursing facilities, hospitals, and even the comfort of patients’ homes via telemedicine. We tailor treatment plans that exceed the needs of diverse groups of patients, ensuring strong core standards of care.

We care for patients struggling with hypertension (high blood pressure), diabetes, metabolic disorders, heart disease, and other chronic diseases, as well as illnesses and injuries.

We offer cardiovascular health care, diabetic management programs, sick visits, comprehensive men’s health care, vaccines, mental health care, and women’s health services, including pelvic exams and Pap smears. We operate on-site labs and COVID-19 testing for our patients’ convenience.

Our office hours are 8am-5pm Monday through Friday. Walk-in hours are 8-11am for established patients. We are closed from noon-1pm daily for lunch.
