He graduated Suma Cum Laude from Indiana Wesleyan University with a Bachelor of Science in Biology. He then received a Bachelor of Science in Medicine from the University of South Dakota. Dr. Stone received his Doctor of Medicine degree from The Ohio State University College of Medicine. He completed his residency in Physical Medicine and Rehabilitation at the University of Pittsburgh where he served as Chief Resident during his final year. He has practiced in Montana, South Dakota, and Tennessee before joining Florida Pain Medicine.

These include nerve blocks, radiofrequency neurotomies, joint injections, spinal injections, spinal cord stimulation, peripheral nerve stimulation, kyphoplasty, and other procedures. Dr. Carlos Diaz is board-certified in physical medicine and rehabilitation and specializes in the nonsurgical treatment of various spine and musculoskeletal problems. He welcomes adults and children over 12 to his practice at Spine, Sport & Physical Medicine Center in Sarasota, Florida. Rather than taking a one-size-fits-all approach to patient care, Dr. Diaz designs treatment plans that meet the needs of the individual.
Every patient can expect a careful review of their medical history and symptoms as well as a detailed medical exam that helps Dr. Diaz create treatment strategies aimed at resolving pain and restoring healthy mobility. Dr. Diaz received his medical degree from Universidad Central del Caribe in Puerto Rico and completed his residency in San Antonio at the University of Texas Health and Science Center. During his training, he worked closely with physicians of multiple specialties, including orthopedic, neurology, and psychology, which gives him a broad scope of medical expertise that greatly benefits his patients.
He leads the team at Spine, Sport & Physical Medicine Center and has gathered a warm and welcoming staff that embraces his passion for treating his patients with respect and compassion, many of whom are fluent in both English and Spanish. Dr. Diaz is happy to provide his community with a fresh, patient-focused perspective regarding orthopedic care.
Dr. Azeem completed his undergraduate education at James Madison University in Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident.
After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Falco.
Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida. He recently developed the first successful non-opioid based interventional pain clinic in the Sarasota, Florida, area.
His background helped shape Dr. Azeem’s approach to treating pain, and he includes both patients and their families in developing treatment plans so that everyone involved understands the origin of the patient’s pain as well as their available treatment options.
Dr. Azeem actively continues to conduct research and author peer-reviewed publications in addition to treating patients, President, Florida Society of Pain & Neuroscience, Chair of Strategic Development for State Society Engagement and Advocacy, American Society of Pain & Neuroscience and Board of Directors, Florida Prescription Drug Monitoring Foundation.

Ashot Kotcharian, MD, is a board-certified physical medicine and rehabilitation (PM&R) physician and interventional pain management specialist at 360 Orthopedics in Sarasota, Venice, and Lakewood Ranch, Florida.
Dr. Kotcharian earned his Bachelor of Science in bio-chemistry and molecular biology at Nebraska Wesleyan University. He then earned his medical degree at the University of Toledo College of Medicine in Ohio. He completed both his internship and residency in PM&R at the Hospital of the University of Pennsylvania before completing an interventional spine and sports medicine fellowship at the same institution.
Dr. Kotcharian previously served a most distinguished career in medicine as Assistant Professor of Physical Medicine and Rehabilitation and as Assistant Fellowship Director of the Interventional Spine and Musculoskeletal Fellowship, both at Johns Hopkins University School of Medicine. He is an expert at both diagnostic and interventional fluoroscopic-guided spinal procedures including steroid injections, nerve blocks, and radiofrequency denervation.
Dr. Kotcharian enjoys outdoor activities with his family and is a “foodie”. He is active in the sport of martial arts since his youth and continues to practice Taekwondo, Brazilian Jiu-Jitsu, and Karate. He proudly achieved and holds the rank of 2nd-degree black belt in Taekwondo. Dr. Kotcharian is fluent in multiple languages including Russian and Spanish. When time allows, he is fond of playing chess, reading, meditation, exercise, and painting.
