Dr. Azeem completed his undergraduate education at James Madison University in Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident.

 

After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Falco.

 

Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida. He recently developed the first successful non-opioid based interventional pain clinic in the Sarasota, Florida, area.

Dr. Warren was born in Kentucky and raised in the heartland of Columbus, Ohio. He attended the Ohio State University where he was president of the Society of Physics Students, and earned his BS in physics. Dr. Warren later attended graduate school at Case Western Reserve University, where he earned an MS in Neuroscience. He attended medical school at the University of Cincinnati, where he was appointed as the Cincinnati Academy of Medicine Liaison and Legislative Representative. He was awarded the Golden Scalpel Award for anatomical expertise, and earned his MD in 2002. In 2008, he completed his residency in Anesthesiology at SUNY Buffalo, during which his training was interrupted while he served a tour of duty for Operation Iraqi Freedom.

After residency, Dr. Warren practiced as an anesthesiologist for over 10 years. During this time, he held several key leadership positions, including Chair of Anesthesiology at Doctors Hospital of Michigan in Detroit, and served on the Pharmacy & Therapeutics Committee at Town & Country Hospital in Tampa. He also completed the Baycare Physician Leadership Course while working at St Joseph’s Hospitals.

His commitment to duty has always been an important part of his life. He served for over 15 years in the US Army Reserve, both enlisted and as a medical officer, having served two tours of duty in Iraq.

After practicing anesthesia for over 10 years, Dr. Warren completed an ACGME-accredited fellowship in Interventional Pain Medicine at the University of South Florida. He is now board-certified in both pain medicine and anesthesiology by the American Board of Anesthesiologists. In 2024, he joined the faculty of USF as an Assistant Professor for the USF Pain Fellowship program. 

Dr. Warren treats a variety of pain conditions including neck and back pain, muscle pain, neuropathic pain, cancer pain, and many other types of pain. Dr. Jonathan Stone is double board certified by the American Board of Physical Medicine and Rehabilitation and by the American Board of Pain Medicine. He graduated Suma Cum Laude from Indiana Wesleyan University with a Bachelor of Science in Biology. He then received a Bachelor of Science in Medicine from the University of South Dakota. Dr. Stone received his Doctor of Medicine degree from The Ohio State University College of Medicine. He completed his residency in Physical Medicine and Rehabilitation at the University of Pittsburgh where he served as Chief Resident during his final year. He has practiced in Montana, South Dakota, and Tennessee before joining Florida Pain Medicine.
