Darshi Edirisooriya, APRN-CNP, is a pediatric nurse practitioner at Pediatric Practitioners of Oklahoma in Claremore, Oklahoma.
Darshi grew up next door in southwest Missouri! She attended Vanderbilt University (VU) in Nashville, Tennessee, where she earned a bachelor’s degree in neuroscience. She continued her academic career at VU and graduated as a primary care pediatric nurse practitioner with a Master of Science in nursing.
She moved to Tulsa and joined Pediatric Practitioners of Oklahoma in 2018. Darshi enjoys providing care to her patients and their families and loves seeing them grow. Smiles and laughs really make her day. She keeps a very special place in her desk for drawings from patients.
Darshi feels she has always been destined to be a pediatric nurse practitioner, and she even dressed as the famous nurse Florence Nightingale for an assignment all the way back in third grade!
In her spare time, Darshi enjoys exploring the outdoors, traveling, cooking, and quality time with friends and family.
Patricia “Trish” Farmer, DNP, APRN-CNP, is a pediatric nurse practitioner at Pediatric Practitioners of Oklahoma. She has been practicing in the Claremore, Oklahoma, area since 2011.
Trish received her nursing degree at Georgia State University in Atlanta in 1987. She gained experience by working in the neonatal intensive care unit, as a home infusion nurse, and as a coordinator/manager with kidney transplants. She then expanded her education by obtaining her Master of Science in nursing at the University of Oklahoma in Norman in 2009. Trish went on to earn her Doctorate in Nursing Practice at Maryville University in St. Louis, Missouri, in 2014.
Trish has dedicated her life to maintaining her pediatric patients’ health. In 2013, she opened Pediatric Practitioners of Oklahoma in Claremore, Oklahoma. She loves working with children and their parents.
In her downtime, Trish enjoys spending time with her family, riding horses, and playing with her dogs.

Melita Tate, MD, is a double board-certified physician in internal medicine and pediatrics and the co-founder of Grassroots Healthcare in Tulsa, Oklahoma.
Known for her holistic approach to medicine, Dr. Tate helps patients of all ages—from infants to seniors—achieve long-term wellness by addressing the root causes of illness. She emphasizes the power of nutrition, lifestyle modifications, and functional medicine to reduce dependency on prescription medications and improve quality of life.
With extensive experience in both chronic disease management and acute care, Dr. Tate specializes in thyroid disorders, diabetes, hormone imbalances, mood disorders, women’s health, and personalized weight management. Her patient-first approach blends evidence-based care with genuine compassion, empowering individuals to take control of their health. She also brings years of urgent care experience from Tulsa’s Springer Urgent Care, where she sharpened her skills in fast-paced, high-stakes medical situations.
An Oklahoma native, Dr. Tate earned her medical degree from the University of Oklahoma College of Medicine and completed her residency in Tulsa. She is currently pursuing her third board certification in functional medicine to further elevate her integrative care model.
When she’s not seeing patients, Dr. Tate enjoys gardening, home-cooked meals with her wife, and cozy evenings with a good book. Together, they share a vibrant life with their nine children and three grandchildren.
