Be Well Primary Health Care Center, LLC is an Article 28 Diagnostic and Treatment Center that is conveniently located in Flatbush, Brooklyn, New York and has been serving the community since 2004. Perla Tate, MD, is a board-certified obstetrician and gynecologist at Vantage Medical Associates, P.C., which has locations in the Bergen Beach, Coney Island, Williamsburg, and Sheepshead Bay neighborhoods in Brooklyn, the Far Rockaway neighborhood in Queens, and Brentwood and Babylon, New York. She specializes in women’s health care, including infertility, ultrasound, colposcopy, and laser surgery.

Dr. Tate completed the pre-medical program at Queens College, City University of New York, in Queens, New York. She attended Universidad Autónoma de Guadalajara School of Medicine in Guadalajara, Mexico, for her medical degree.

Dr. Peter Guirguis is the leader in the New York Metropolitan Area as well as abroad with patients traveling from afar for Minimal Invasive and Robotic Procedures for a variety of complex causes of pelvic pain secondary to uterine fibroids, ovarian polyps, endometriosis, abnormal uterine bleeding. Dr. Peter has been asked to share his expertise on various minimal invasive gynecological techniques including both preoperative and postoperative care within the united states and most recently abroad. 

As a proud advocate of educating women to empower them in their health care decisions, board-certified OB/GYN Peter Guirguis, MD, FACOG, provides personalized care to women of all ages. He is a fellow of the American College of Surgeons and a Fellow of the American College of OB/GYN, proud to serve women in Park Slope, Bay Ridge, and Staten Island. A Brooklyn native, “Dr. Pete” is the son of the founder of The Guirguis Obstetrics & Gynecology Group, Dr. Fayez Guirguis.

Dr. Guirguis, also affiliated with New York Presbyterian Brooklyn Methodist Hospital and Staten Island University Hospital, has over a decade of experience in health care. He received his undergraduate degree from Tufts University in Boston, and continued his education at SUNY-Upstate Medical University in Syracuse, New York, where he earned his medical degree.

From there, Dr. Guirguis went on to complete his OB/GYN residency at the University of Connecticut, affiliated with Hartford Hospital, and then joined the Guirguis Group practice in July 2011. Dr. Pemberton is board-certified by the American Board of Obstetrics and Gynecology. Dr. Kakossian graduated Medical School with Honors and Distinction in 1991. She completed an exchange fellowship program at Columbia University, NY in Reproductive Endocrinology and Infertility. She graduated as Chief resident in Obstetrics & Gynecology at Lutheran Medical Center, NY.

Dr. Kakossian has done extensive research in Laparoscopic and Hysteroscopic Surgery for different gynecologic conditions and produced many publications in this field. She was trained by and operated with the professor and founder of Laparoscopic Surgery Dr. H. Reich at Columbia University, NY. She lectured at International Conferences of Gynecologic Surgeons in Las Vegas, Miami, Montreal, and Atlanta and demonstrated the latest advances in Laparoscopic surgery in Moscow, Russia.

I graduated from the Midwifery Program at SUNY Downstate Medical Center. Prior to becoming a midwife, I was an Ob/Gyn sonographer. Upon graduation from Downstate, I received multiple awards for dedication and perseverance to the Midwifery Program. I worked for a very busy private Ob Gyn practice in the last three years and will continue to be determined and dedicated to keeping the well-being of women, their babies and family members foremost in my mind.

Caroline Paster graduated from the Midwifery Education Program at SUNY Downstate Medical Center, where she received the Spirit of Midwifery Award along with Clinical and Academic Excellence Awards. Caroline received her Bachelor’s Degree in Neuroscience and Women’s Studies from Syracuse University and completed a post-baccalaureate at New York University. Caroline previously worked as a midwife at Coney Island Hospital servicing a diverse population of patients. She was recently awarded the 2016 Outstanding Preceptor Award from the American College of Nurse Midwives. Caroline thoroughly enjoys her work in obstetrics and gynecology and is also involved in training student midwives.

Edward Chan, MD, of Mott Women Healthcare is a compassionate and diligent OB/GYN serving the Chinatown community of New York City. He is board-certified by the American Board of Obstetrics and Gynecology. Dr. Chan has more than 20 years of experience as a physician, allowing him to provide expert care to his patients. 

Dr. Chan began his medical education in Illinois, where he received his medical degree from the Loyola University Chicago Stritch School of Medicine. He then traveled to New York to complete his internship and residency at Saint Vincent Catholic Medical Center. Compassionate double board-certified physician, Susan Wolf, MD, offers high quality care and a patient-centered approach to medicine at the University Reproductive Associates Hasbrouck Heights and Hoboken locations in New Jersey.  

Before joining University Reproductive Associates in 2008, Dr. Wolf was the physician in charge of the Division of Reproductive Endocrinology & Infertility at Beth Israel Medical Center in New York. 

Dr. Wolf graduated with a Bachelor of Arts from Bryn Mawr College in Pennsylvania before completing her medical degree at the Icahn School of Medicine at Mount Sinai in New York City. Mohamed Esiely, MD is a board-certified obstetrician and gynecologist serving the Hoboken community at his practice Health Wise Women in New Jersey.

Before immigrating to the United States, Dr. Esiely was a physician in Egypt where he received his medical degree from Cairo University. As an attending physician, he focused his skills on keeping students knowledgeable as they studied, lectured, and traveled the world.

In the United States, Dr. Esiely completed his residency at New York’s Mount Sinai School of Medicine, where he was chief resident. He became a certified practitioner of the da Vinci® surgical system in 2012, a robotic system used to perform minimally invasive surgical procedures.

Dr. Esiely established Health Wise Women in 2010. In his more than 20 years practicing medicine, he has received numerous awards, including the Laparoscopic Surgery Award and Resident Award for Excellence in Teaching. The Egyptian Ministry of Health also selected Dr. Esiely for the Family Planning Program Award, and he won first prize for a case report at New York Medical College’s Annual Resident Research Day.

Daniel F. Roshan, MD, FACOG, FACS, specializes in maternal-fetal medicine, demonstrating an expertise in high-risk pregnancies that’s well known in New York City and beyond, as women needing chorionic villus sampling are referred to him from across the country. He truly considers it an honor to take exceptional care of his patients with high-risk pregnancies at his practice, Rosh Maternal & Fetal Medicine in the Midtown East area of New York City.

Shira Plonka is a board certified Genetic Counselor at ROSH Maternal Fetal Medicine. Additionally, she is a member of the National Society of Genetic Counselors (NSGC); she earned her Master of Science in Human Genetics from Sarah Lawrence College. Prior to receiving her master’s, Shira received an honors degree in psychology and social work from University of Waterloo. Shira has held internships in many of New York City’s top hospitals, acquiring a broad exposure to many disciplines and specialties, including cancer and pediatric genetics. 

Shira focuses on preconception and prenatal genetic counseling: carrier screening, prenatal genetic screening and diagnostic testing. She provides information about the risks of chromosome abnormalities, birth defects, genetic conditions, and teratogen exposure to individuals, couples or families. Ellen received her undergraduate degree in molecular biology from Skidmore College in Saratoga Springs, NY. She earned her masters degree in Physician Assistant Studies from SUNY Upstate Medical University in Syracuse, NY.

Upon completing a post graduate Emergency Department Fellowship, she transitioned into OBGYN to follow her passion for Obstetrics and Women's Health. She places emphasis on health education, preventative care, diagnosis, and treatment of women to have a positive impact on their lives.

Ellen is originally from Upstate New York. In her free time she enjoys playing field hockey, hiking, cooking, traveling, and spending time with family.
