Nathan Jent is a certified Physician Assistant at Michigan Avenue Primary Care, a multispecialty practice located in The Loop of downtown Chicago, Illinois.
Nathan earned a Bachelor of Health Sciences degree at Butler University in Indianapolis in 2019. During his time at Butler, Nathan was heavily involved on campus working as a new student orientation guide and an executive chair for Dance Marathon, a large peer-to-peer fundraising initiative focused on raising money and awareness for children's medical care, including cancer research and maternity support. He then went on to earn his Master of Physician Assistant Studies degree in 2021, also at Butler University. Nathan remains a Bulldog fan and enjoys watching his alma mater on the basketball court (Go Dawgs!).
Nathan started his career in orthopedics, and he continues to enjoy seeing patients for a variety of orthopedic complaints including joint pain, athletic injuries, and nutrition and fitness. His interests in medicine also include helping patients achieve their goals, big and small, through pro-lifestyle changes and medical management.
Nathan enjoys working at Michigan Avenue Primary Care for several reasons, namely getting to meet and lend a helping hand to a variety of patients while also getting to work with a team of such kind, talented, and highly motivated people.
In his free time, Nathan enjoys spending his time playing golf, weight training, adventuring the city with his spouse, and home cooking nutrient-rich and delicious foods. He also loves dogs and experiencing new things whenever possible.
Dr. Barrett attended Virginia Tech for his undergraduate degree, studying biochemistry. He earned his medical degree at Eastern Virginia Medical School and completed his residency at the University of Illinois at Chicago.
Dr. Barrett’s treatment philosophy is one of empowering the patient: “Patients have the right to be heard and to partner with me to create a treatment plan appropriate for their individual needs and situation”. His clinical interests are primary care, preventative medicine and LGBT health.

Lester Montesa, MS, PA-C, is a dedicated physician assistant at Michigan Avenue Primary Care in The Loop neighborhood of Chicago, Illinois. With a strong educational background and extensive experience, Lester provides exceptional primary care to patients of all ages.
Lester earned a Master of Science in Physician Assistant Studies from Rush University in Chicago. During this advanced medical track, Lester mastered physical exam skills and gained expertise in diagnosing and managing various medical conditions in pediatric and adult populations. His ability to build strong rapport with patients and family members ensures a compassionate and comprehensive approach to healthcare.
Before this, Lester completed a Bachelor of Medical Science at Dominican University in River Forest, Illinois, and a Bachelor of Science in Psychology with a minor in Neuroscience from Loyola University in Chicago, where he was recognized on the Dean's list for his academic excellence.
In addition to his education, Lester has valuable work experience as a senior medical assistant at an immediate care/primary care facility and as a medical volunteer in orthopedic surgery for RUMC Global Health in San Juan de la Maguana, Dominican Republic. These roles have equipped him with practical skills and a deep understanding of patient care in diverse settings.
Lester holds a Physician Assistant Certification from the NCCPA and is an active member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants.

Jocelin Gamez, PA-C, is a certified physician assistant at Michigan Avenue Primary Care, a multispecialty practice located in The Loop of downtown Chicago, Illinois.
Jocelin earned her Bachelor of Science in biology with a minor in psychology at the University of Illinois at Chicago. During her undergraduate years, she simultaneously worked as a certified pharmacy technician and certified emergency medical technician. She then earned her Master of Science in physician assistant studies at the University of Iowa (Go Hawkeyes!). Her interests include women’s health, providing healthcare to Spanish-speaking populations, and preventative medicine.
Jocelin loves that every day is different and that she gets to meet and build relationships with all kinds of people. She loves the continuity of her education through the ever-changing medical advances. She also enjoys being able to teach her patients while also learning from them.

Anne-Sophie Kim, MD, is a dedicated primary care physician at Michigan Avenue Primary Care located in The Loop of downtown Chicago, Illinois. She is committed to patient-centered medicine and provides comprehensive and preventative primary care.
Skilled in managing both acute and chronic conditions, Dr. Kim aims to improve the overall health of her patients. Her particular areas of interest include managing chronic diseases such as diabetes, obesity, dyslipidemia, hypertension, and thyroid diseases.
Dr. Kim completed her residency in Internal Medicine at the University of Chicago – NorthShore in Evanston, IL. Joseph Rabi, MD, is a pain management and rehabilitation physician practicing at The Neuroscience and Spine Institute, located in Orland Park, Illinois.
Dr. Rabi completed his undergraduate coursework at DePaul University in Chicago, Illinois, in 2006. He then went on to earn his Doctor of Medicine from St. George's University School of Medicine in Grenada. Dr. Rabi completed his internal medicine internship through the University of Illinois at Chicago at Advocate Christ Medical Center before completing his physical medicine and rehabilitation residency through the University of Chicago at Schwab Rehabilitation Hospital.
An active contributor to the field, Dr. Rabi has helped to author numerous publications, research projects, abstracts, and presentations. He has a special interest in treating work/personal injuries, electrodiagnostics, opioid misuse, spine pain, concussions, migraines, regenerative medicine, and chronic pain.
To stay up to date with the latest advancements in medicine, Dr. Rabi maintains memberships with the American Academy of Physical Medicine and Rehabilitation, the American Society of Regional Anesthesia and Pain Medicine, the North American Pain Society, and the American College of Physicians.

William Crevier, MD, is a board-certified internal medicine and pediatrics physician at The Center for Primary Healthcare in Orland Park, Illinois. He is also the president and medical director of the practice and cares for patients of all ages: from infants through adults.
Dr. Cevier grew up in Vermont. He attended the University of California at Berkeley, graduating magna cum laude with a degree in Biochemistry. He received his medical degree from the University of Chicago and completed his residency at Loyola University Medical Center in Maywood, Illinois. He’s an advocate for access to care and served as the founding executive director of two Christian community health centers.
Dr. Crevier sincerely enjoys getting to know his patients. He gives each and every patient the time and attention they deserve in a warm, welcoming atmosphere.
