I have been a board-certified Physician Assistant with Lake Grove Family Practice for over 7 years now and I have loved watching the practice grow into what it has become and what the future holds. Along with being a Federal Aviation Administration (FAA) Senior Aviation Medical Examiner, I am trained to provide flight physicals and evaluations for the FAA. As a pilot myself, I enjoy working with other airmen. I have been a licensed pilot since 1974. I am an Aviation Medical Examiner (AME) who is certified by the FAA to provide all class physicals, including private pilot, air traffic control, and commercial/airline pilot medical certification. I have done extensive training in mental health and drug/alcohol recovery. As a result of these trainings and certifications, I am currently the only Human Intervention Motivation Study (HIMS Independent Medical Sponsor) IMS (Independent Medical Sponsor) in Oregon. HIMS is an occupational substance abuse treatment program specific to pilots.
When I am not spending time with patients and the office, you can find me raising my granddaughter with my wife, Debbie. In addition to raising three children, I spend my free time enjoying photography, white water rafting, scuba diving, and of course flying.

It is such a pleasure to be part of the team here at Lake Grove Family Medical. I was born and raised in California and have trained in many different places all over the country. I received my medical degree from St. George’s University in Grenada, West Indies and I completed my residency in family medicine at Virginia Commonwealth University (VCU) affiliated BON Secours St. Francis Family Medicine Residency program.
My specialties include full spectrum family medicine and have had previous experience managing low- risk prenatal care. I love caring for all ages and have always had a special interest in preventive care, mental health, and women’s health. Eric Blake, ND, Dipl. Ac., is an expert naturopathic physician and the medical director of Portland Clinic of Holistic Health, providing care in Downtown Portland, Oregon. Amy Meoli, MD, is a board-certified pulmonary disease doctor at Own Sleep Medicine in Kansas City, Missouri, and Vancouver, Washington.
Dr. Meoli completed her bachelor’s and medical degree at the University of Missouri-Kansas City in Kansas City, Missouri. As a board-certified family physician known for his kindness, active listening, and helpfulness, Warren Okuns, MD, practices at Southwest Family Physicians in Tigard, Oregon. Dr. Okuns earned his medical degree in 2005 at the University of the West Indies in St. Augustine, Trinidad and Tobago. In 2006, he completed his rotating medical internship at Port of Spain General Hospital, located in Port of Spain, Trinidad and Tobago.
