Dr. Lee earned his medical degree from the Universidad Autónoma de Guadalajara in Zapopan, Mexico, and New York Medical College in New York City. After graduation, he completed a residency at the University of Wisconsin Hospital and Clinics in Madison, Wisconsin.

Before moving to Waxahachie, Texas she lived in Killeen, Texas to support her military spouse. She worked in the community as a house call nurse practitioner. She also provided primary care as a nurse practitioner in Mansfield, Texas.
Zayba Hoda, PA-C, and Certified Health Coach, has been a Physician Assistant since 1999. With over 20 years of experience in primary care, she is excited to now also be providing health coaching services for the benefit of her patients at Family Care and Wellness.
Zayba earned her Bachelor of Science in Biology and Psychology from Florida State University, and her Master's in Physician Assistant Studies from the University of Florida. She recently received her certification in health coaching to focus her attention on helping clients fulfill their healthcare goals. She has especially enjoyed the areas of diabetes care and assisting patients in quitting smoking.

Tariq Mallick, MD, MBA, is the board-certified family medicine physician at Family Care & Wellness in Allen, Texas.
